「Submission」

1.	Entry form may be downloaded from the official website (http://www.myvideocompetition.com) or from any of the organizing or sponsoring agency’s website, or may be picked up in person at the organizer’s office.

2.	All entries must be received no later than May 31, 2008. Complete package should include one DVD and two additional copies of the short, 4 stage photos (in 4R or JPEG format), and 150-word description. The complete package may be hand delivered or mailed to the organizing committee postmarked by 5p.m. May 31, 2008 at the latest.
